Ordering Workflow

Step 1: Patient Information

Search for patient by name, date of birth or MRN. If the patient is found, select the patient. If the patient is not found, click the ‘+’ button to create a new patient.

When creating a new patient, complete the basic patient information. The five required fields on the page are: First name, Last name, DOB, Sex, MRN. Once completed, click ‘Save’ to return to the order process.

For hospital customers, ‘No Insurance on file’ will be checked automatically to minimize workload. To add the patient’s insurance information, uncheck the box. Click ‘Order History’ to see historical orders for the patient and ‘View Full Record’ to see all patient information.

Step 2: Test Selection

Verify the selected test is correct. If the selected lab accepts compassionate care/medicaid pricing, check the box to document the appropriate price. To add or modify the product, type your search term in the search box and use the search tools to find the requested.

Order Details

Complete all appropriate fields. The only required field on this page is the provider name. Once the provider is selected, the ‘Continue’ button will populate. Click when ready to proceed.

Select Indication You can select the reason for ordering the test(s) from the dropdown. This is not a required field, so if you don't see your targeted response, you can leave the field blank and manually write the indication on the requisition.

Specify Test Order When Ordering Multiple Tests When ordering more than one test, you can indicate whether your preference is to run the tests sequentially or concurrently. When only a single test is ordered, this field will not be available. When multiple tests are ordered, the default setting will be sequential. If selecting sequential, you should use the up/down arrows to arrange the tests in the order you want them performed. On the test order page of the requisition, a corresponding box will be checked to indicate sequential/concurrent, and if sequential has been selected, the tests will be numbered based on the final order.

Hospital Status Select whether the patient's status at time of order is Inpatient or Outpatient.

ICD-10 Codes Complete the ICD-10 codes field if desired. If entering multiple codes, please separate them with a comma.

Test Notes Users can make special requests about the test or include additional test related information here. The information populated in this section will appear on the test requisition form in the 'Test Notes' section. Please note clinical information related to the patient should be entered in the 'Clinical Notes' section on the History step.

Edit Custom Panels For laboratories that have the Custom Panel feature turned on, you can customize your panel within the Tests to Order section. To add a gene to a custom panel, type in the gene name then select the gene from the dropdown. You can add as many genes as identified in the parenthesis (in the example below the number of genes is 20). This policy is set by the laboratory. To remove a gene, click the X next to the gene name. Once the order is placed, the additional genes will be added to the order page of the requisition and will be found in the Details section of the test orders on the Dashboard.

Category-Specific Information For certain categories that require additional information, Whole Exome and Known Mutation Analysis, additional optional fields will be made available for completion. The information entered in these fields will populate on a special test requisition page.

Billing

Select the appropriate billing method.

For institution billing, your Concert Platform administrator will complete this information as part of the account setup process, and it will automatically populate when selected. The Account Number field applies to any account numbers you have with reference labs, and will change automatically depending on which lab you order from.

If institutions maintain multiple institution billing accounts, users can select from a list by clicking the dropdown under the 'Billing Option' and selecting from the appropriate institution.

For insurance billing, users have two options. First, if you usually complete all requested insurance information on the requisition, you can fill out the fields and the data will automatically populate on the requisition. You can populate information for both primary and secondary insurances, if applicable. If your normal workflow doesn't involve filling out the insurance information and you simply include a copy of the front and back of the insurance card, you can leave the fields blank and just click 'Continue.'

Please note if patient contact information is required as part of the insurance submission, you can complete that information on the 'Patient' step.

For Self Pay, the platform doesn't collect or process credit card information, so you should select Self Pay, click 'Continue' and complete the requested information on the printed requisition.

For tests where the laboratory is not billing your institution or the patient's insurance for the test order, you can select the 'No Charge' option, which will enable you to filter out those tests when analyzing the data within the Administrative Reports.

Step 6: Submit and Manage

Preview the requisition form to ensure all fields are completed correctly. Once done, click ‘Submit Order’ button to initiate order. For hospitals using the internal review process, the button will say ‘Submit for Review’. To print the requisition, click the ‘Print’ icon in the status bar.
